Stakeholder Advisory Panel (SAP) Selection Form
Appointing Individuals To PQA SAPs


The SAP appointment process is online and is submitted by the PQA member organization's key contact.

Each PQA member organization can appoint up to SIX (6) individuals to participate on the SAPs, with a maximum of two (2) appointees per SAP.

Previous PQA measure development teams, task forces and panels have been highly successful based on the exceptional skills and experience of appointees. Please consider matching the interests and talents of your appointees when making a SAP selection.
 
Steps to appoint an individual to a PQA SAP:
 
Step 1: Choose your PQA member organization from the drop down menu below.
Step 2: Enter the first  appointee name and contact information
Step 3: Enter SAP selection & Area(s) of expertise for the appointee
Step 4: Indicate whether appointee is from your organization or from a non-member organization
Step 5: If applicable, repeat Steps 2-4.
Step 6: Enter the name of the person filling out this online form
Step 7: Submit all of your appointees
